Update, Tim Gordon from Jerald Sulky never bothered to get back to me after his email said he would last week. Let this be a warning to everyone. They won't charge you for something they don't send but if you are putting a project on hold waiting for a tire you will be sitting around forever.

Park Tools has a nice head that can mount like that. Pricey but will last forever. My stand is over 35 years old and still going strong.
