i think the way to do it is remove or break the locking mechanism of the gears if it works like the shimano shifter, add a spring so it comes back and your done!!!
 
I started to do one with a stick shifter from a Spyder 5-speed. I used a standard shifter cable since the little lead ball at the end is already made to fit in the shifter. I had the bike about 75% done when I sold all the parts to fund another bike.
 
Would this be cabled to a drum brake hub or a caliper brake?

It would be cool to be able to pull back on the shifter for the front and push forward for the rear.

I had a 10 speed stem mount shifter that had a front and rear lever. It was an older friction shift. I used the left lever for the front brake and the right for the rear derailer (bike was a 2 speed) lever didnt spring back so you had to pull it back when you slowed down or stopped. It is possible to do
 
ya know, ya wouldn't even need a return spring.

this little norco used a 3 speed thumb shifter as a brake lever.

1st gear brakes where off
2nd gear mild brake
3rd gear brake on.
the brakes would only release when you took it outta 3rd gear.
