I got this Stem shifter on E-bay a couple months ago, I need a little help with getting the correct freewheel/ derailleur for it. I am using it on a New Schwinn DelMar , waiting for Monarch repop forks to arrive, ordering disc brake/freewheel wheels this week. Stripping it down to bare frame ard a complete re-do. I know its not Ratty, but there is a lot of knowledge here.

That seems like alot of modifications.

To add the gears with the shifter your best best would be to find a old donor bike for the wheel and rearderailer. Buying new will set you back more $$$.

Disc Brakes are probably the hardest modification to get done. You will need new wheels front and back with rotors, calipers and brake levers. Your frame does not have disc brake mounts so you would have to figure that out. ChoppersUS makes some adapter mounts that could work with some additional fabrication. The rear disc mount would probably have to be welded. Rim brakes could equally be as hard depending especially on the repop Monarch fork. The frame may not have a brake bridge for mounting the rear brake.

The repop Monarch forks are notorious for not being strong enough. The welding on the plates is not sufficient.

These frames have a mounting point for long arm center mount sidepull brakes, they even have a geared version. For disc, see the previous post, unless of course you have experience with that. I definitely wouldn't trust the repop Monarch with a brake (especially disc) unless the crown is fully welded, they don't come that way. An old 10 or 15 spd 26" mtb or 5-spd cruiser with intact drive may be your best bet for drive train parts, which would also save you the hassle of figuring spoke length.
