I see a lot of people run those 3 1/2'' wide rims. Wonder how they stop the chain from rubbing the side of the tire? Without a jack shaft does the chain run at a really radical angle? Do you have to space the crank out farther or something?

I believe they have wide cranks too. I know the junk Schwinn OCC Chopper I picked up has a very wide bottom bracket and long crank shaft to get around the 4" tire

Look for an OCC Schwinn on the side of the road or at the dump and cut out that bottom bracket. It'll be 3 piece, which I know you are not fond of.

Basically, if you're running a single chainring up front, you can use some spacers to give you a little more clearance on a standard width bottom bracket. The OCC chopper BB has a very wide crank axle, but uses standard cups... you can buy the crank axle separately if you can make or find a wide 3-piece BB shell. You'll have to put some thought into rear hubs too, most won't clear a 3" or wider tire without an offset sprocket, etc... choppersus.com sells cheap singlespeed wide rear hubs, Sturmey makes an extra wide 3-speed rear hub with a disk brake.
