How can I fix this issue on my kustom kruiser?

Fat washer between the sprocket and the right side bb cone.

Dished cogs for coaster brake hubs or spacers that can be installed on the inside or outside of the cog. Note that Shimano & Sturmey archer cogs have a slightly different hole size. The larger cog can slip on the smaller driver if you pedal hard. The smaller cog can be filed to fit the larger driver.

Or a 3 piece conversion bb for 3 piece cranks. The new crank axle would need to be long enough. Most conversion kits don't come in widths so that's pretty much trial and error. some 3 piece cranks have more offset than others.

Ultimately the sprocket and cog should be in the same geometric plane so the chain is less likely to jump off.
