Sorry, no answers to any of your questions but this is one of the coolest bar set ups I've ever seen. It's not my bike but I love the old vintage racing bicycle look.

And it's actually ridable!

Dude, them bars are the dogs! Anyone got any ideas where I can find bars like those, the oldschool curly drops in steel...... nice nice :D.

Find a set of Soma Major Taylor bars . they are steel and look very close to these. I put a set on an old track bike I have and they look like they are right out of the twenty`s
Here a link http://store.somafab.com/majortaylorbar.html

I believe those bars are lauterwassers. They are similar to track bars but the drops are spread out making the bars a little shallower and a little wider.

jOe posted some absolutely gorgeous bikes, but I have a different opinion on the bar height. I'd say as far as looks go, you need to keep the tops below seat level, or you are going to look like a fool (just my ex-racer opinion). But really, what's the point of having dropped bars mounted sky high?

while i don't agree with everything grant peterson espouses even a stopped clock is right twice a day. keep in mind some of us here are making fit assumptions based on quality, properly fitting bar/stem combos- typical old 70's short/narrow 'ten speed' setups just will not fit most adults properly.

having said that, the 'point' of a rivendell inspired setup with 'sky high' (really only a few centimeters) bars is to make a drop bar practical for riders without a racing pedigree. a slightly higher bar with bent elbows is less tiring and more efficient than a too-low bar with arms locked straight. the goal is a variety of hand positions for everyman, the lowest of which can be ridden comfortably for miles into a headwind if necessary.

the 'fool' is the recreational rider who only wears out his bar tape on the tops because he's insecure about not looking like a racer. you paid for an entire handlebar, you may as well set it up so you can use it
