I would think the thumbscrew goes through there, and pulls on the outside rear of the dropout. Just guessing. :?

Although it appears to have been made for a frame with rear-facing dropouts, if you google "surly chain tensioner" you can find info and pictures about it being installed with some success even on frames with forward facing dropouts such as those found on cruisers. But if you're just trying to keep the axle from sliding forward some good serrated washers under well tightened axle nuts will more simply and cheaply solve that problem.
