From the seat tube decal I'm thinking it might be a Schwinn American.

Looks like the forks may be tweaked a little...any good bike shop can straighten them for around $20.

i work with a guy who use to be a bicycle mechanic and he was trained my an old school bike gent who was like 80 years old and taught him a neat trick on how to forks and i am going to giv it a whirl when i have time.......

i am pretty sure the bike has a serial # i just have not had enough time to go over it......
