You can pick up one for cheap at Harbor Freight (LINK). Pretty useful to have around, I've used mine several times through the years.

Head is on the inside. Drill and tap fork and thread bolt all the way throuigh then you can cut/grind anything that sticks out on the outside.

I would just drill from the outside of the fork,

A dumb mistake I made was not giving my welder an axle spacer to hold the fork blades in place. Because of that the heat from welding the fender mount warped one of the legs in.

So I put my spreader on it.
