While at it I had to take off the shifter and took some pictures while assembling it again. I thought some of you might be interested...
Basic part is a long bolt that sits in the hole from the original rod brakes mechanics.

The shifter itself is mounted on a piece of 22mm tube that I rounded a bit to get a snug fit to the frame

This all sits on the bolt

And is pulled tight with a dished washer and a regular nut. The dished washer helps to keep it centered and to avoid wiggling.

The shifter handle I made from a long threaded bolt, a small piece of tube and a piece of the original pedal rubber. The shiny sleeve tubes come from an antic camera lens extension.

The head of the grip bolt is filed to fit in-between the raised parts of the shifter knob.

The handle and sleeve is then pressed on and centered with the backcover of an old pocket watch and an acorn nut to finish it off.
